Where you identify gaps against any of these principles, remediate them before signing, or be prepared to describe a credible plan and timeline for closing them, since the Charter is a public declaration that NHS customers and the wider sector may hold you to.
Once you've confirmed your position, complete the NHS England declaration form to formally register your organisation's commitment to the Charter, and keep your underlying practices (particularly your DSPT rating) up to date, since the Charter's credibility depends on continuing to meet these commitments over time rather than a one-off sign-up.
